About the position

The General Case Manager at Families in Transition of Santa Cruz is responsible for providing comprehensive case management services to families in need, focusing on housing stability and retention. This role involves conducting assessments, advocating for clients, and linking them to community resources while ensuring compliance with program guidelines. The Case Manager will work closely with families to develop individualized plans that address their unique challenges and promote long-term housing retention.

Responsibilities

  • Assess and screen participants to determine eligibility in accordance with program guidelines.
  • Provide housing search supports to participating families including assisting family to identify housing preferences and projected post-exit rent limits.
  • Assist families in identifying their needs, clarifying options and developing goals.
  • Linkage to community resources which promote long-term housing retention.
  • Provide outreach and education on program services to community programs as needed.
  • Act as client advocate in securing appropriate housing, employment, benefits, education, counseling, health care, childcare, transportation, treatment and other needed services.
  • Development of comprehensive housing service plans that promote housing retention, address individual challenges and promote achievable goals.
  • Facilitate rental agreements with landlords in scattered site rental units and ensure clients understanding of rental agreements and lease compliance.
  • Development of individualized temporary phased rental subsidy plans.
  • Offer, as appropriate and applicable, Peer Counseling.
  • Offer skill building and tools which promote successful transitional budgeting as families move from homelessness to housing stability.
  • Offer monthly budgeting skills and tools to promote housing retention post-exit.
  • Extensive record keeping, data entry and documentation of services.
  • Coordination of services with community based and government social service providers.
  • Attend case reviews, meetings, trainings, workshops and conferences related to position.
  • Attend and participate in a minimum of 2 agency sponsored fundraising/outreach events each year, as assigned.
  • Review Summarized data and provide meaningful contribution to program and agency evaluation activities.
  • Responsible for maintaining confidentiality at all times.
  • Responsible for completion and submission of reports as assigned and needed.
  • Responsible for submitting completed rental assistance requests accurately and promptly.
  • Responsible for data entry and client status updates by specified due dates as assigned.
  • Maintain good communication and work as a team with other staff and service providers.
  • Maintain clear and concise records.
  • Perform Home Visits monthly once a family is housed.
  • Follow all agency and program policies and procedures.
  • Other duties as assigned.

Requirements

  • Experience in addiction counseling and behavioral health.
  • Proficient in intake processes and case management.
  • Mid-level fundraising experience.
  • Strong budgeting skills.

Nice-to-haves

  • Experience in landlord advocacy and negotiations.
  • Knowledge of community resources for families in transition.
