About the position

The Direct Support Professional (DSP) plays a crucial role in providing oversight, guidance, and training to individuals with disabilities, helping them achieve their maximum potential and promoting independence and community integration. This position involves organizing activities, ensuring proper supervision, maintaining communication with families, and documenting services accurately. The DSP is also responsible for assisting with personal care and health needs, participating in team meetings, and adhering to agency policies.

Responsibilities

  • Organize and schedule individual goals and/or activities based on need and frequency of the Staff Action Plan, Life Plan or schedule of services.
  • Provide necessary supervision for individuals as specified in their Life Plan, Staff Action Plan, Safeguard Plan, and LOS.
  • Maintain close communication with individuals and/or families regarding any scheduling changes.
  • Provide supports, assessment, and training to persons supported according to their written Staff Action Plan (SAP).
  • Ensure necessary level of supervision (LOS) is met by following the individual's Safeguard Plan and other applicable plans.
  • Accurately document services contemporaneously in Electronic Health Record (EHR) or with paper notes as applicable.
  • Treat all protected health information consistent with HIPAA.
  • Maintain accurate records by collecting, compiling, evaluating data and submitting in a timely manner.
  • Provide input into the development and implementation of the SAP or service plan in accordance with the Life Plan.
  • Maintain knowledge and understanding of individual's medical, physical, psychological and dental health care needs.
  • Recognize and respond in a timely manner to signs and symptoms of illness/injury and medical emergencies.
  • Provide personal hygiene and health care assistance to people supported as needed.
  • Provide training and encouragement to people supported to ensure good personal health care as needed.
  • Become certified in any required training and maintain certification on an annual basis.
  • Participate in developing and delivering remote services to individuals as necessary.
  • Assist individuals in receiving needed/desired medical services and communicating/documenting pertinent information as per Agency policy.
  • Support the safety of all individuals in everyday situations.
  • Work as a team to support positive behavioral strategies.
  • Provide transportation to people supported as necessary and follow proper safety procedures.
  • Drive Agency vehicles, complete necessary paperwork and report any accidents or maintenance issues immediately.
  • Assist people supported to build positive relationships in their community.
  • Support people to make personal choices and to advocate for themselves.
  • Communicate professionally with co-workers, people supported, families and the community as necessary.
  • Complete all required Agency Training per agency policy and apply knowledge and skills gained to job.
  • Demonstrate professionalism in the workplace including demeanor, appearance, attention to punctuality and attendance policies.
  • Report for duty as scheduled and check employee email/schedule each shift.
  • Attend and participate in meetings, as requested.
  • Adhere to all Agency and Program Policies and Procedures.

Requirements

  • High school diploma or equivalent.
  • Valid NYS driver's license with a safe driving record per agency policy.
