About the position

The GOALS Program Manager at People's Community Clinic is responsible for managing the Generating Outcomes And Liaisons for Students (GOALS) Program, which supports Pediatric, Adolescent, and Young Adult patients facing academic and neurodevelopmental challenges. This role involves ensuring compliance with organizational standards, fostering relationships with local schools, and overseeing patient communication and case management to optimize patient success.

Responsibilities

  • Support Pediatric, Adolescent, and Integrated Behavioral Health Departments with services for patients aged 4-21 with academic and neurodevelopmental concerns.
  • Build and maintain relationships with local schools and district representatives to increase awareness of the program.
  • Initiate GOALS Program enrollment through the clinic's electronic health record.
  • Educate patients and their families about the GOALS program through outreach and in-clinic meetings.
  • Contact schools, therapy agencies, and other healthcare centers to obtain additional records and information related to patient care.
  • Oversee patient and family communication and case management, including explaining the GOALS program and facilitating appointments.
  • Manage bimonthly Psychiatry Conference by ensuring provider coverage and appropriate follow-ups.
  • Oversee organization, tracking, and scoring of GOALS assessment scales.
  • Lead the GOALS Community Advisory Council by selecting topics and facilitating meetings.
  • Plan monthly GOALS staffings and programmatic meetings and take meeting minutes.
  • Assist with planning and orientation of GOALS staff or health professional trainee onboarding.
  • Provide presentations about the GOALS Program to enhance awareness and knowledge.
  • Identify opportunities for improvement in program service delivery and workflow.
  • Track GOALS patients through routine metric reports and report metrics to staff as needed.
  • Serve as Clinical Process Owner for ADHD Medication Management and improve adherence to NCQA standards.
  • Work with various teams to identify opportunities for funding and reimbursement.
  • Supervise and manage assistants, interns, or volunteers.

Requirements

  • College degree in Social Work, Education, Public Health, Vocational Nursing, Community Health Work, or related field.
  • Minimum of two years of experience in social, medical, education, human services, or community-based settings.
  • Bilingual in English/Spanish preferred.
  • Superb interpersonal skills to interact with diverse individuals.
  • Ability to think critically and solve problems effectively.
  • Excellent communication skills, both oral and written.
  • Computer literacy, including Microsoft Office (Word and Excel) and information security protocols.
  • Skills in developing and managing electronic reports and processes.
  • Knowledge of electronic health records preferred.
  • High-level organizational and time management skills.
  • Competence in recruitment, supervision, and management of assistants, interns, and volunteers.
