About the position

Join the SNF group in Grand Junction, Colorado, where we manage several Skilled Nursing Facilities (SNFs). We are seeking dedicated Primary Care Physicians (PCPs) to apply for this rewarding role. As a PCP in our group, you will have the opportunity to travel to various sites, with a maximum of four locations, ensuring that your work remains dynamic and engaging. We pride ourselves on offering the highest compensation for SNF work, with a base salary of $250,000 and the potential to earn up to $300,000 based on performance and productivity. In this position, you will experience very minimal overnight call coverage, allowing for a better work-life balance. We understand the importance of time off, which is why we offer 20 days of paid time off (PTO) annually. Additionally, we provide three days off for Continuing Medical Education (CME) along with a stipend of $2,000 to support your professional development. To ensure your health and well-being, we contribute $3,600 per year towards your health insurance, with three plans available for you to choose from. Our benefits package includes a 401k plan with a 3% match, up to a maximum of $6,000. We also offer 100% coverage for employee dental and vision insurance, as well as short-term disability and basic life insurance. For long-term disability, we provide 50% coverage. Furthermore, we cover malpractice insurance and offer relocation assistance, ensuring a smooth transition to your new role. Responsibilities

  • Provide primary care services to patients in skilled nursing facilities.
  • Travel to a maximum of four sites to deliver care.
  • Participate in minimal overnight call coverage.
  • Manage patient care and coordinate with nursing staff.
  • Document patient interactions and maintain accurate medical records.

Requirements

  • Medical degree (MD or DO) from an accredited institution.
  • Board certification in Family Medicine or Internal Medicine.
  • Active medical license in Colorado or ability to obtain one.
  • Experience in skilled nursing facility care is preferred.
  • Strong communication and interpersonal skills.

Nice-to-haves

  • Experience with geriatric patients.
  • Familiarity with electronic health record (EHR) systems.
  • Bilingual skills are a plus.
