About the position

The Habilitation Specialist 1 (NY HELPS) position involves coordinating residential habilitation services for individuals with developmental disabilities. The role includes developing and reviewing individualized plans, ensuring compliance with regulations, and providing training to staff. The specialist will work closely with treatment teams to support the needs and goals of individuals in various group homes.

Responsibilities

  • Attend all annual and semi-annual ISP/lifeplan meetings for approximately 20-30 individuals in an assigned case load.
  • Develop Residential Habilitation Plans/Staff Action Plans based on valued outcomes and IPOP.
  • Review and revise all Res Hab. Plans or Staff Action Plans every six months or as needed.
  • Ensure all Res Hab. Plans and staff action plans data sheets are completed and set up new data sheets monthly.
  • Upload accurate documents into HER and secure hard copies of required documentation.
  • Write monthly progress notes based on each individual's response to interventions.
  • Provide training to DSA Residential staff on all Res Hab. Plans or Staff Action Plans.
  • Conduct AM and PM observations to ensure implementation of Res Hab. Plans and active engagement.
  • Ensure continuity between Res Hab. Plan/staff action plans and Day Hab. Plan.
  • Review all ISP/Life-plans for accuracy and communicate corrections to the MSC/Care Coordinator.
  • Work directly with team members to develop ISP/Life Plan and establish valued outcomes.
  • Responsible for record keeping and record review to ensure OMIG Compliance.

Requirements

  • Permanent service as a Habilitation Specialist 1 OR a bachelor's or higher-level degree in a human services field.
  • One year's experience working with people with a developmental disability and a Bachelor's Degree or higher in a human services field for QIDP designation.

Nice-to-haves

  • Experience in developing and implementing individualized plans for individuals with developmental disabilities.
  • Knowledge of compliance regulations related to habilitation services.
