About the position

The Harm Reduction Clinic Nurse Care Manager (HRC NCM) plays a crucial role within the multidisciplinary Office-based Addiction Treatment (OBAT) team at Brockton Neighborhood Health Center's Harm Reduction Clinic. This position focuses on providing comprehensive care for patients with opioid and alcohol use disorders, facilitating their treatment journey through assessment, education, and coordination with various healthcare providers. The HRC NCM is responsible for delivering trauma-informed care and ensuring that patients receive the necessary support and resources throughout their treatment process.

Responsibilities

  • Conduct initial patient assessment and intake for treatment with buprenorphine or naltrexone.
  • Assess and monitor patients in the induction, stabilization, and maintenance phases of treatment.
  • Administer extended-release intramuscular naltrexone and subcutaneous buprenorphine to patients.
  • Provide ongoing care management, education, and support for patients in all phases of treatment.
  • Follow federal and state guidelines in providing care to opioid and/or alcohol dependent patients.
  • Complete timely and appropriate documentation and record keeping in the electronic health record.
  • Contribute to required State OBAT reporting and paperwork.
  • Educate patients and their families about buprenorphine or naltrexone therapy, including risks and benefits.
  • Involve patients in the development of their treatment plans.
  • Identify community resources and promote access or referral to them.

Requirements

  • Current Massachusetts Registered Nurse license.
  • CARN certification preferred, must obtain within 2 years of employment.
  • 2-3 years of experience working with homeless or underserved adult populations.
  • 1+ years of experience in substance use disorder treatment, harm reduction, or behavioral health.
  • Excellent customer service skills.
  • Strong verbal and written communication skills.
  • Proficiency with electronic health records and strong computer literacy.

Nice-to-haves

  • BSN preferred, may substitute BSN for 1 year of experience.
  • OBAT training preferred, must attend within 1 year of employment.
  • Multilingual candidates strongly preferred.
