About the position

The HCBS Programmer role at Imagine the Possibilities Inc. is focused on providing administrative support in a Home and Community-Based Services (HCBS) setting. This position aims to empower individuals with functional, cognitive, and mental health needs by facilitating their access to services that allow them to live independently. The programmer will be responsible for monitoring services, developing strategies to achieve individual goals, and coordinating care in a manner that respects the needs and desires of the individuals served.

Responsibilities

  • Create and maintain team culture through active involvement and monthly meetings.
  • Provide coaching opportunities for staff and fill in for the team as needed.
  • Monitor and schedule vehicle usage for services and oversee budgets for individuals served.
  • Support individuals in communication with their Representative Payee Coordinator and manage financial documentation.
  • Monitor medication check-ins and appointment scheduling for individuals served.
  • Serve as the primary contact with the Medicaid Community Based Care Manager (CBCM) for individuals served.
  • Input accurate information into the Electronic Medical Record (EMR) and develop individualized plans with the CBCM and Interdisciplinary Team (IDT).
  • Conduct quarterly reviews of goal progress for each individual served.
  • Communicate timely with individuals served, their families, and case managers regarding service changes.
  • Document interactions and reasons for changes in services provided.
  • Collaborate with the IDT to write support goals and personal outcome plans within 21 days of new individuals being entrusted to care.

Requirements

  • High school diploma or GED equivalent.
  • Valid driver's license and reliable transportation.
  • Strong communication skills.
  • Ability to adapt and learn in a changing caregiving environment.
  • Confidence in advocating for individuals while being self-aware.

Nice-to-haves

  • Experience in a caregiving or supportive services role.
  • Familiarity with Medicaid and community-based care systems.
