About the position

The Health and Wellness Navigator at Rose Senior Living Carmel plays a crucial role in guiding residents, family members, and caregivers through health and wellness transitions. This position focuses on ensuring residents receive appropriate levels of care and supportive services, fostering emotional and psychosocial support, and building trustful relationships with residents. The Navigator collaborates with community teams to identify and recommend services, serving as a liaison and advocate for residents' needs.

Responsibilities

  • Ensure residents are in the appropriate levels of care (Independent, Assisted, Memory Care, Respite, Skilled Nursing, and Rehab) and receiving necessary supportive services.
  • Collaborate with community team members to identify and recommend additional services or transitions for residents with changing needs.
  • Promote internal services to enhance resident retention.
  • Act as a resident liaison and advocate, coordinating assessments and providing assistance with internal and external resources.
  • Interact with residents and family members when changes in condition necessitate additional services or moves within the continuum of care.
  • Prepare closing documentation and internal communication in a timely and accurate manner.
  • Develop and update the community move-in guide as needed.
  • Serve as a backup for inquiry calls, walk-in tours, and scheduled appointments.
  • Participate in marketing events as directed by the Director of Sales & Marketing.
  • Maintain personal work areas and ensure they are presentable.

Requirements

  • Two to three years of experience in senior housing or a healthcare-related field.
  • One year of experience in hospitality preferred.
  • Strong interpersonal skills to motivate and encourage residents for maximum independence and quality of life.
  • Excellent verbal and written communication skills for diverse populations, including persons with disabilities and Dementia.
  • Ability to work collaboratively with community management and communicate effectively with residents, families, physicians, and staff.
