About the position

The Health Care Case Manager (Registered Nurse) position at Rutgers School of Nursing in Newark, NJ, is responsible for providing nursing case management services to support the health care and protection needs of children in out-of-home placements. This role involves collaboration with the Department of Children and Families (DCF) and other health care providers to ensure that children receive appropriate health care, including assessments, health plan development, and ongoing monitoring.

Responsibilities

  • Assist with enrollment of a child in a managed care entity and identification of a medical home upon entry into out-of-home placement.
  • Assess a child's health care status and needs by retrieving a complete health history and reviewing medical records.
  • Schedule an Initial Physical Health Evaluation and ensure that all recommendations for health care are completed.
  • Develop a child health care plan in collaboration with the child's medical home and involved specialists.
  • Facilitate transfer of care and records to an identified medical home when the child is changing placement or upon reunification.
  • Conduct nursing placement assessments on children entering or changing out-of-home placement.
  • Provide assistance during workplace emergencies and perform CPR as needed.

Requirements

  • A Bachelor's Degree in Nursing or a BA/BS degree in a health-related field required; may substitute years of pediatric nursing experience for degree.
  • A minimum of 1-2 years of experience in pediatric nursing required.
  • Current licensure as a Registered Nurse is required.
  • Current certification in American Heart Association Basic Life Support for Healthcare Providers is required.
  • Vehicle and a valid license required for off-site visits.

Nice-to-haves

  • Two (2) years of experience in nursing case management, community health nursing, or nursing care of vulnerable families preferred.
