Responsibilities

  • Utilize applicable clinical training and knowledge to provide appropriate health support and solutions to members with chronic health conditions
  • Evaluate the needs of the member by performing telephonic assessments to determine clinical risk and readiness to change
  • Provide telephonic support and coaching for members on ongoing wellness care plans and wellness programs to improve member's health and lifestyle through outreach and education
  • Assist members in developing attainable wellness goals through plans, support, encouragement, and resources
  • Monitor wellness plans and/or member status and outcomes for changes in care and provide recommendations to care plan based on member needs and issues identified to reduce health risks
  • Engage and influence members in decisions relating to achieving and maintaining an optimal healthy lifestyle
  • Interact with healthcare providers as appropriate to ensure that wellness plans are aligned with overall medical plans of care
  • Collect, document, and maintain member information to ensure compliance with current state and federal accreditation standards
  • Provide feedback to leadership on opportunities to improve and enhance care and quality delivery for members in a cost-effective manner
  • Perform other duties as assigned
  • Comply with all policies and standards

Requirements

  • Requires an Associate's degree and 2 - 4 years of related experience
  • Registered Dietitian certification
