Health Coach

About the position

The Health Coach position at Elevance Health is a virtual role focused on providing health coaching to members with chronic diseases. The coach will conduct behavioral assessments, coordinate health coaching plans, and monitor member progress to help achieve health objectives. This role requires collaboration with nurse case managers to ensure effective intervention and support for members' health needs.

Responsibilities

• Conduct behavioral assessments to identify individual member knowledge, skills, and behavioral needs.
• Coordinate specific health coaching as directed by nurse case manager to address objectives and goals identified during assessment.
• Implement coaching plans using behavior change principles to identify member barriers and develop strategies to overcome them.
• Provide feedback to nurse case manager on member goal attainment and clinical issues.
• Monitor and evaluate interventions and modify as necessary.

Requirements

• BA/BS in an appropriate field of specialization (e.g., Health Education, Exercise Physiology, Respiratory Therapy, or Dietician).
• Minimum of 3 years of related experience in health education, exercise instruction, or patient education, or equivalent combination of education and experience.
• Certification, advanced certification, and/or license appropriate to field of specialty as required.

Nice-to-haves

• Prior experience in health coaching and disease management.
• Knowledge of behavioral and/or clinical assessment techniques.
• Experience in health and/or patient education and behavior change techniques.
• Understanding of disease management principles.
