About the position

The Health Data Specialist position at I Am Boundless, Inc. is a full-time role focused on gathering, monitoring, and reporting outcomes data related to the OhioRISE CME. This position is integral to supporting individuals with intellectual and developmental disabilities and behavioral health challenges, ensuring that all required activities and reporting are completed for the population served. The role offers the flexibility of remote work while maintaining communication with care coordination and management teams.

Responsibilities

• Gather, monitor, and report outcomes data related to the OhioRISE CME.
• Ensure all required activities and reporting are complete for individuals in the population pool.
• Work with assigned care coordinators to address any gaps in data reporting.
• Engage with care coordination and management team members via telephone, video conferencing, and electronic communication.
• Interact with staff from other provider organizations or community organizations to obtain or exchange necessary records or documentation.

Requirements

• High School diploma or GED required.
• One to three years of experience working with health data, data analytics, health outcomes reporting, or data reporting tools.
• Excellent communication skills, both oral and written.
• Excellent organizational skills with the ability to stay focused and meet deadlines.
• Ability to prioritize multiple tasks.
• Intermediate computer skills with the ability to learn new software and reporting systems.
• Professional telephone etiquette.
• Ability to show tact and diplomacy when dealing with parents, clients, peers, and staff.
