About The Position

The Health Educator will promote, maintain, and improve individual and community health by assisting individuals and patient groups in adopting healthy behaviors. This role involves collecting and analyzing data to identify community needs and planning, implementing, monitoring, and evaluating health education programs. The Health Educator will also create and implement health education events and initiatives for the community, serving as a resource for other professionals and collaboratives.

Requirements

  • Bachelor's degree in Health Promotion, Health Education, Public Health, or a related field.
  • Minimum of two years of experience in community health or health education.

Nice To Haves

  • Master's degree in Health Promotion, Health Education, Public Health, or a related field.
  • Certified Health Education Specialist (CHES) certification.

Responsibilities

  • Develop implementation strategies to improve the health of Hamilton patients.
  • Assess the needs of Hamilton patients and develop programs to address these needs.
  • Provide support and guidance to patients and their families for quality health education services.
  • Conduct group education sessions for patients and their families.
  • Partner with Hamilton providers and external agencies to meet patient care objectives through health education.
  • Collaborate with other departments to establish quality health education events and initiatives for the community.
  • Provide individualized, client-centered health education to patients and/or family members.
  • Develop age-specific and culturally appropriate health education messages and materials.
  • Facilitate patient learning groups on topics such as nutrition, physical activity, tobacco cessation, reproductive health, and disease prevention.
  • Collaborate with Community Health Workers to deliver health education to target populations.
  • Assist patients in navigating the health care system.
  • Collect and analyze patient outcome data, providing reports as needed.
  • Manage program objectives, interventions, and outcomes; assess impact on target population.
  • Conduct routine evaluations of programs and recommend improvements.
  • Participate in task forces or committees addressing health education program needs.
  • Perform other duties as assigned.
