About the position

The Health Navigator at Jefferson Healthcare is a full-time role focused on providing compassionate patient care and support. The position involves establishing trusting relationships with patients and their families, coordinating community referrals, and acting as a patient advocate. The Health Navigator will work closely with the Care Coordinator to evaluate patient and family status, ensuring effective communication and support.

Responsibilities

• Establish trusting relationships with patients and their families.
• Provide general support and consistent communication to the Care Coordinator.
• Coordinate community referrals and act as a patient advocate.
• Evaluate patient/family status in collaboration with the Care Coordinator.

Requirements

• High School degree or equivalent required.
• BLS certification required.
• MA or LPN preferred.
• Previous medical/clinical experience preferred.
• Experience working in a community-based setting for at least 1 to 2 years preferred.
• Community Health Worker formal training required or completion of Jefferson Healthcare sponsored training within the first 90 days of employment.
• Good communication skills, including listening and appropriate language use.
• Ability to provide emotional support, encouragement, and motivation to patients.
