About the position
The Health & Wellness Coach Case Manager at Bert Nash Community Mental Health Center is responsible for providing client-directed interventions to help participants achieve success in various life areas, including housing, careers, relationships, and health. This role involves coaching clients to identify their motivations, recognize their skills, and overcome challenges, while working as part of a whole-person centered treatment team. The position emphasizes the importance of health and wellness across multiple life domains and requires engagement with community partners and crisis intervention when necessary.
Responsibilities
- Utilize coaching principles to provide client-directed interventions.
- Help clients find motivation and recognize their skills.
- Focus on clients' goals and assist them in overcoming challenges.
- Serve adult clients receiving community-based services.
- Attend Wellness Life Coach training and pass required exams.
- Encourage clients to implement healthy life practices across various domains.
- Employ motivational interviewing and client-directed approaches.
- Respond to referrals and engage in initial client service contact.
- Maintain professional relationships with community partners and agencies.
- Assist with crisis intervention and crisis planning as needed.
Requirements
- BA/BS degree or four years of equivalent education and/or experience in human services.
- Reliable transportation and a valid driver's license with proof of insurance.
- Prior computer experience and basic computer fluency.
- Ability to work independently and collaboratively with others.
- Flexibility in assigned working hours.
- Commitment to interdisciplinary and community mental health approaches.
