The role involves assisting with daily living activities, monitoring residents' health, and participating in care planning, all while fostering a respectful and compassionate atmosphere.

Responsibilities

• Provide personal assistance and activities of daily living (ADL) for residents according to their support plan.
• Answer resident call signals promptly.
• Assist residents with toileting and personal hygiene needs.
• Deliver meals and ensure residents' nutritional needs are met.
• Observe and record residents' conditions, including food and liquid intake, output, and vital signs, reporting any changes to wellness staff.
• Complete necessary documentation to maintain residents' health status.
• Participate in planning resident care through shift reports and staff meetings.
• Stay current with state and facility required annual training.
• Perform other duties as assigned.

Requirements

• High school diploma or GED equivalent.
• Minimum of one year of experience in personal and/or long-term care.
• Current First Aid and CPR certifications preferred.
• Strong interpersonal and communication skills.
• Ability to read, speak, and understand English at a 12th grade level or higher.
