Valley Health is seeking a dedicated Advanced Practice Provider (APP) to join our Heart Failure Clinic located in Winchester, Virginia. This full-time position offers a unique opportunity to work in a dynamic environment focused on the care and education of heart failure patients and their families. The APP will play a crucial role in facilitating patient care from the initial contact through the entire continuum of heart failure management. This includes assessing patient needs, developing care plans, and providing education to patients and their families about heart failure management and treatment options. The ideal candidate will have experience in cardiology or a strong interest in the heart failure specialty. While experience is preferred, we are open to considering candidates who demonstrate a passion for this field and a commitment to providing high-quality patient care.
