About the position

Valley Health is seeking a dedicated Advanced Practice Provider (APP) to join our Heart Failure Clinic located in Winchester, Virginia. This full-time position offers a unique opportunity to work in a dynamic environment focused on the care and education of heart failure patients and their families. The APP will play a crucial role in facilitating patient care from the initial contact through the entire continuum of heart failure management. This includes assessing patient needs, developing care plans, and providing education to patients and their families about heart failure management and treatment options. The ideal candidate will have experience in cardiology or a strong interest in the heart failure specialty. While experience is preferred, we are open to considering candidates who demonstrate a passion for this field and a commitment to providing high-quality patient care. The position is designed to be a supportive role within a busy clinic, allowing the APP to focus on patient interactions without the demands of weekend work or on-call responsibilities. Responsibilities

  • Facilitate care and education for heart failure patients and their families.
  • Assess patient needs and develop individualized care plans.
  • Provide ongoing management and follow-up for heart failure patients.
  • Educate patients and families about heart failure management and treatment options.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.

Requirements

  • Current certification as a Nurse Practitioner or Physician Assistant.
  • Experience in cardiology or a strong interest in heart failure specialty preferred.
  • Ability to work collaboratively in a team-oriented environment.
  • Strong communication and interpersonal skills.

Nice-to-haves

  • Previous experience in a heart failure clinic or cardiology setting.
  • Familiarity with the latest advancements in heart failure treatment.
