About the position

The Health Coach position within the Division of General Internal Medicine (DGIM) focuses on supporting the Extended Intervention for Tobacco Use (EXIT) study, which aims to enhance access to cessation services and reduce tobacco use among individuals experiencing homelessness. Health Coaches will engage clients through telehealth sessions, helping them set and achieve wellness goals related to tobacco cessation and overall health, while addressing co-occurring mental health and substance use issues.

Responsibilities

  • Motivate and engage clients to set and achieve their wellness goals through telehealth sessions.
  • Utilize a process-oriented coaching approach to help participants define their vision and set SMART goals.
  • Reinforce and maintain participants' visions through empowerment and skill-building activities.
  • Facilitate interactions between participants and pharmacists or primary care providers regarding medication management.
  • Participate in skills-based training and learning collaboratives with the study team.

Requirements

  • Bachelor's degree in a related area or equivalent experience/training.
  • Experience working with individuals experiencing homelessness and those with mental health and substance use disorders.
  • Basic knowledge of health promotion and education.
  • Effective interpersonal skills to work with a diverse client group.
  • Ability to communicate effectively both verbally and in writing.

Nice-to-haves

  • Basic technological skills (e.g., Zoom, smartphone applications).
  • Familiarity with health coaching and patient-centered healthcare.
  • Coaching and motivational interviewing skills.
  • Knowledge of health conditions related to tobacco use.
  • Experience with mindfulness meditation and cognitive-behavioral therapy.
