About the position

The Home Health Aide (HHA) / Personal Care Assistant (PCA) position at Ideal Home Care Services involves providing quality in-home care to clients of all ages. The role focuses on implementing care plans, ensuring client safety, and fostering positive relationships while delivering both skilled and non-skilled services. Responsibilities

  • Ensure quality and safe delivery of home health care services.
  • Implement client's plan of care in accordance with the Agency's process and procedures policy.
  • Participate in Client's case conferences according to the Agency's policies and regulations.
  • Submit updates and relevant information regarding the Client's plan of care to the Registered Nurse in a timely manner.
  • Work with patients of all ages that require care in their homes.
  • Carry out patient care plan activities.
  • Accompany patients to doctor's offices or hospital visits.

Requirements

  • High School Diploma or Equivalent - Preferred.
  • Home Health Aide certification required.
  • 1 year Home Health Care experience.
  • Ability to read, understand, and execute instructions.
  • Ability to communicate and write effectively.
  • Ability to work and deal within a stressful environment.

Nice-to-haves

  • Bilingual - Spanish/Creole/etc. (Preferred - Not Required) - Must also be able to read/write and speak English.
