The HIIM Data Integrity Analyst - Validation plays a crucial role in ensuring that medical records and forms are accurately identified and indexed within the established application. This position involves identifying and validating all required documentation, and if requested, verifying that all documents have been authenticated before the record is filed as complete. The Analyst will also perform re-analysis to ensure that all documents are assigned appropriately and are accurate. Essential duties include indexing all scanned documentation within the medical record to the appropriate facility, patient, and document type, reviewing scanned documentation for image quality, and identifying documents that need to be re-scanned due to poor image quality. The Analyst will also verify the correct scanning of the medical record number (MRN), account number, and document type, ensuring the integrity of the legal medical record. Daily communication with the facility HIMD regarding validation status and addressing questions related to validation completion are also key responsibilities. The role requires the ability to work independently as a remote employee and may include weekend coverage.

Responsibilities

  • Indexes all scanned documentation within the medical record to the appropriate facility, patient, and document type.
  • Reviews all scanned documentation for image quality, including page orientation, coloring, and legibility.
  • Identifies documents needing to be re-scanned due to poor image quality.
  • Identifies missing pages within the chart that may require additional follow-up.
  • Adheres to established CHS standards/policies and system workflow guidelines to add and re-assign accounts to appropriate work queues for processing and/or follow-up.
  • Verifies correct scanning of the MRN, account number, and/or document type.
  • Verifies quality and correctness of all scanned documents from the imaging vendor and the facility.
  • Ensures the integrity of the legal medical record.
  • Communicates daily with the facility HIMD on the validation status.
  • Communicates via established protocols to local HIM director of missing documents and other needs requiring local facility intervention.
  • Addresses appropriate questions related to validation completion from the HIIM Data Integrity Quality Manager, HIIM Supervisors, and/or Facility HIMDs.
  • Actively performs validation on medical records within 12 hours of availability, excluding established facility exceptions.
  • Meets or exceeds corporate established Medium productivity standards for validation within 6 months of hire.
  • Meets or exceeds corporate quality standard of 98% or greater on all validated medical records with an overall critical error accuracy of 100%.
  • Assists in cross-training other employees as necessary.
  • Demonstrates competency of all validated assignments within 60 days of hire.

Requirements

  • High School Diploma required.
  • Three (3) years' experience in medical record analysis required.
  • Experience with electronic medical record imaging systems - Cerner Millennium preferred.
  • Advanced analytical skills with the ability to problem solve and identify potential workflow barriers/issues.
  • Strong written and verbal communication skills.
  • Knowledge of Cerner imaging software, Google Suite, Microsoft Office applications, database software, and audio/video conferencing.

Nice-to-haves

  • Previous experience in an HIM Department or office environment strongly preferred.
  • Ability to decipher different handwriting types.
