About the position

The HIMS/Referral Coordinator (Medical Records Clerk) plays a crucial role in managing and maintaining patients' health information, ensuring accuracy and confidentiality in medical records. This position involves coordinating patient referrals to specialists, scheduling appointments, and supporting health information management in a primary care setting focused on seniors. The role requires on-site work and emphasizes compliance with privacy regulations while facilitating effective communication between patients and healthcare providers.

Responsibilities

  • Assemble and maintain patients' health information in medical records and charts.
  • Ensure all forms are properly identified, completed, and signed.
  • Enter necessary information into the electronic health record (EHR) system.
  • Communicate with physicians and staff to clarify diagnoses or obtain additional information.
  • Assign codes to diagnoses and procedures as needed.
  • Maintain and update patient medical records in accordance with HIPAA and practice policies.
  • Monitor patient charts for completeness, accuracy, and compliance with legal standards.
  • Assist with document management including scanning, indexing, and archiving medical records.
  • Perform regular audits of patient records to ensure data integrity.
  • Assist with coordination of patient referrals to specialists and healthcare providers.
  • Communicate referral details to patients, including appointment times and instructions.
  • Track and manage referral statuses to ensure timely follow-up and appointment scheduling.
  • Verify insurance coverage to obtain necessary authorizations for referrals.
  • Assist with scheduling patient appointments efficiently and managing the appointment calendar.
  • Confirm patient appointments and send reminders as appropriate.
  • Provide general administrative support to the front office team.

Requirements

  • 1 year of experience in medical records, health information management, and referral coordination, preferably in a primary care setting.
  • 1 year of experience with computer systems or demonstrated technical aptitude.
  • Experience with electronic health records (EHR) systems and medical coding procedures.
  • Familiarity with medical terminology and ICD-10 codes.
  • Experience with scheduling software.
  • Strong knowledge of HIPAA regulations and healthcare privacy standards.
  • High level of accuracy in data entry and record management.
  • Proficiency in Microsoft Office Programs (Word, Excel, Outlook) and medical office software.

Nice-to-haves

  • Associate's or Bachelor's Degree in a related field.
  • Possession of certifications such as RHIT, CEHRS, CMSC, CPCS, CPCC, CHAA, or CMAA.
  • Value Based Care experience including knowledge of HEDIS, CPT/ICD coding, and CAHPS/HOS Patient Experience.
  • Experience in Primary Care, Internal Medicine, Family Medicine, Urgent Care, and/or Occupational Health.
