About the position

The HIV Outreach Specialist will play a crucial role in conducting outreach and providing support to individuals living with HIV or those at risk of HIV infection. This position focuses on educating clients about sexual health, assessing their needs, and linking them to necessary resources and services. The specialist will engage with the community to raise awareness and promote health initiatives related to HIV prevention and care.

Responsibilities

  • Conduct outreach and assist persons living with HIV or those engaged in behaviors that put them at risk of HIV infection.
  • Identify, screen, and enroll eligible clients.
  • Assess and educate clients and individuals at risk on sexual health and behaviors that are high risk for HIV.
  • Increase individual knowledge about HIV/STI/HCV transmission, prevention and care, as well as access to healthcare.
  • Actively outreach and engage clients in comprehensive assessments to identify barriers and/or unmet needs.
  • Develop an action plan to eliminate or reduce these barriers.
  • Provide immediate interventions and linkages to testing and/or additional supports that are identified at the first meeting.
  • Educate clients on health and human service resources.
  • Link clients to Options' current programs serving PLWH/A including housing, financial assistance, and Ryan White funded services for longer term support.
  • Maintain contact with referral sources to identify program candidates.
  • Complete intakes and gather supporting documentation.
  • Maintain and update status of clients.
  • Outreach/attend health fairs/community forums and other public events.
  • Confidently educate on sexual health and high-risk behaviors and interventions to the general public and with identified individuals at risk.
  • Complete assessments and reassessments as needed.
  • Participate in quality improvement activities as appropriate.
  • Intervene in situations requiring immediate attention to ensure safety of clients.
  • Other program or administrative activities as appropriate.

Requirements

  • Bachelor's degree in healthcare or human services and at least 2 years of qualifying experience.
  • A Master's degree in healthcare or human services may be substituted for 1 year of experience.
  • Valid driver's license, safe driving record, access to a vehicle and valid auto insurance is required.
  • Verifiable full or part-time case management or case work with persons with HIV/AIDS, and/or persons with a history of mental illness, homelessness, chemical dependence, chronic illnesses, or other populations of persons in need.

Nice-to-haves

  • Bilingual preferred (English/Spanish speaking).
