About the position

The HIV Prevention Services Coordinator plays a crucial role in enhancing the quality and accessibility of HIV prevention and care services within the St. Jude Neighborhood Health Centers. This position is responsible for managing projects related to HRSA's Ending the HIV Epidemic Initiative, focusing on improving screening and prevention efforts for HIV/AIDS in underserved communities. The coordinator will engage with patients, providers, and stakeholders to develop effective workflows, educational materials, and performance metrics to ensure successful program implementation and patient satisfaction.

Responsibilities

  • Facilitate St. Jude Neighborhood Health Centers' participation in HRSA's Ending the HIV Epidemic Initiative.
  • Develop workflows, protocols, and standards of care related to HIV prevention and care.
  • Plan, coordinate, and report on performance related to HRSA's grant initiative.
  • Engage in HRSA-related meetings and maintain rapport with HRSA staff.
  • Partner with the HIV Prevention Team to screen and assess patients for HIV risk.
  • Develop and disseminate educational materials on HIV prevention and treatment.
  • Deliver education to caregivers, providers, and patients regarding HIV prevention and treatment.
  • Monitor organizational performance on quality measures related to HIV prevention and care.
  • Organize and document internal project meetings.
  • Identify and monitor metrics of project performance and prepare presentations on outcomes.
  • Administer HIV screenings, tests, and risk assessments to patients.
  • Provide education on HIV prevention, treatment, and resources to patients.

Requirements

  • Master's Degree in Health Education or Public Health.
  • Minimum of two years' experience with Health Education and/or project management.
  • Commitment to working with underserved patient populations.
  • Computer proficiency in Word, Excel, and Visio.
  • Fluency in English and Spanish.

Nice-to-haves

  • Experience using Epic Systems electronic medical record.
  • Knowledge of HIV prevention and treatment best practices.
