About the position

The HIV Support Specialist-Community Health Worker (HIV-CHW) plays a crucial role in supporting individuals with HIV/AIDS by providing education, peer support, and assistance in navigating the healthcare system. This position involves working closely with healthcare providers to bridge gaps in care, promote health literacy, and connect clients to necessary services both inside and outside of clinical settings.

Responsibilities

  • Coordinate needed services for people with HIV/AIDS.
  • Provide trust-based information, education, and peer support.
  • Help patients cope with and overcome fear related to their health status.
  • Assist clients in navigating the healthcare system by scheduling appointments and providing reminders.
  • Make referrals to transportation, housing, mental health care, or substance use treatment.
  • Conduct one-on-one or group education sessions regarding HIV and medications.
  • Document and track patient care and referrals.
  • Motivate and encourage clients to obtain care and other services.
  • Connect and communicate with hard-to-reach populations.
  • Participate in local and community events to increase awareness and engagement.

Requirements

  • High School diploma or GED required; A.A. Degree or Bachelor's preferred.
  • Typing and computer skills required.
  • Prior telephone experience required.
  • Flexible, open-minded, knowledgeable, resourceful, and self-motivated.
  • Access to reliable transportation is required.
