About the position

The Home and Community Support position at Millcreek of Pearl involves providing essential assistance to individuals with daily living activities. This role is crucial in ensuring the health and well-being of eligible individuals by helping them with various tasks such as eating, meal planning, bathing, dressing, and maintaining personal hygiene. The position also requires light housekeeping duties that directly relate to the individual's health, including bed-making, dusting, and vacuuming. In addition to personal care, the role includes assisting individuals in accessing community resources, shopping, and attending appointments. Accurate documentation is a key responsibility, as employees must complete and submit Daily Activity Logs and Contact Summaries promptly. Adhering to safe transportation procedures is essential, as is following the appropriate protocols for incident reporting. The position requires flexibility in scheduling to meet the needs of families and maximize the use of authorized hours. Employees are expected to attend training sessions and adhere to policies regarding personnel updates. Maintaining confidentiality and a positive rapport with individuals and their families is vital. The role also involves keeping supervisors informed about relevant information regarding service provision and adhering to all safety policies and worker's compensation guidelines. Other duties may be assigned as necessary, making adaptability an important trait for candidates.

Responsibilities

  • Provide assistance with activities of daily living, including eating, meal planning, bathing, dressing, and hygiene/toileting.
  • Assist with light housekeeping needs related to the health and well-being of the individual, such as bed-making, dusting, and vacuuming.
  • Help individuals access community resources, shopping, and appointments.
  • Accurately complete and submit Daily Activity Logs and Contact Summaries in a timely manner.
  • Adhere to safe transportation procedures at all times.
  • Follow appropriate protocol for completion of Incident and Serious Incident reports.
  • Work a flexible schedule based on the family's needs to maximize authorized hours.
  • Attend trainings and in-services as requested and adhere to personnel update requests in a timely manner.
  • Adhere to UltiPro usage policies, procedures, and timelines.
  • Follow Activity Plan and/or family instructions regarding personal care of the eligible individual.
  • Keep supervisor informed of pertinent information relevant to service provision.
  • Maintain a positive attitude and rapport with the eligible individual and family.
  • Adhere to confidentiality policies and procedures.
  • Refrain from excessive and unwarranted absenteeism and/or tardiness.
  • Adhere to all Millcreek and HCBS waiver program policies and procedures.
  • Follow all safety policies and adhere to all worker's compensation program guidelines.
  • Perform other duties as assigned.

Requirements

  • High School diploma or GED is required.
  • A valid Driver's License is required.
  • At least one year of working experience is required.
  • One year of experience working with children/adults with developmental disabilities is preferred.
