As a Home Health Registered Nurse, you will participate in ongoing interdisciplinary assessments of patients, completing initial and ongoing comprehensive assessments of their needs. You will be involved in the development and evaluation of the plan of care in partnership with the patient, their representatives, and caregivers. Your role will include clarifying mutually agreed-upon goals with the patient and incorporating these into the service plan. You will provide services ordered by the physician as indicated in the plan of care, counsel and educate patients and families on healthcare-related needs, initiate appropriate preventative and rehabilitative nursing procedures, and refer to other services as needed. Monitoring and evaluating patient care outcomes, communicating changes in patient needs to appropriate staff, and supervising paraprofessional staff are also key responsibilities. You will effectively communicate with other members of the interdisciplinary healthcare team to promote coordination of patient care and participate in the Quality Assessment and Performance Improvement (QAPI) program.
