As a Home Health Registered Nurse, you will participate in ongoing interdisciplinary assessments of patients, completing initial and ongoing comprehensive assessments of their needs. You will be involved in the development and evaluation of the plan of care in partnership with the patient, their representatives, and caregivers. Your role will include clarifying mutually agreed-upon goals with the patient and incorporating these into the service plan. You will provide services ordered by the physician as indicated in the plan of care, counsel and educate patients and families on healthcare-related needs, initiate appropriate preventative and rehabilitative nursing procedures, and refer to other services as needed. Monitoring and evaluating patient care outcomes, communicating changes in patient needs to appropriate staff, and supervising paraprofessional staff are also key responsibilities. You will effectively communicate with other members of the interdisciplinary healthcare team to promote coordination of patient care and participate in the Quality Assessment and Performance Improvement (QAPI) program. Responsibilities

  • Participates in ongoing interdisciplinary assessment of the patient.
  • Completes initial and ongoing comprehensive assessments of the patient's needs.

  • Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient care.
  • Communicates with all physicians involved in the plan of care and other health care practitioners related to the current plan of care.
  • Participates in Quality Assessment and Performance Improvement (QAPI) program.
  • Participates in in-services or training, as requested.

Requirements

  • Graduate of an accredited school of nursing and current RN License in SC or compact license.
  • 2 years' experience as a nurse with 1 year of experience in home health.
  • Proficient computer skills and workplace software knowledge.
  • Ability to work with people, function well under stress, use clinical judgment, be creative and accept challenges.
  • Able to effectively deal with multiple changes based on patient needs and scheduling.
  • Clinical proficiency in nursing, strong communication skills, and compassion.
  • CPR Certification.
  • Valid driver's license and proof of insurance.
