About the position

The Home Care Service Coordinator is responsible for scheduling and supervising in-home care workers and clients within a designated geographic area. This role is essential for ensuring that older adults and individuals with disabilities receive the necessary support to live safely at home. The position offers a full-time schedule from Monday to Friday, with training provided for the right candidate.

Responsibilities

  • Schedule employees according to the client's care plan established upon intake.
  • Process patient authorizations and communicate with central admissions, ensuring chart preparation for all new clients.
  • Create and manage work schedules, accommodating changes due to client requests, illness, or other absences.
  • Provide alternate coverage to ensure uninterrupted client services.
  • Supervise direct service employees, setting expectations for attendance, performance, and conduct.
  • Assist with the new hire process, ensuring all documentation is completed accurately and timely.

Requirements

  • High school diploma or equivalent required.
  • Strong interpersonal, organizational, and communication skills.
  • Experience in recruiting and covering shifts in the field as needed.
  • Proficiency in computer skills, including Microsoft Word, Microsoft Excel, and scheduling programs.
  • Must have reliable transportation, including a valid Driver's License, auto insurance, and a vehicle.
