About the position

The Home Care Nurse position involves providing compassionate and dedicated care to patients in their homes. The role requires close collaboration with patients, their families, and healthcare professionals to ensure comprehensive care is delivered effectively, focusing on patient assessments, personal care, and education.

Responsibilities

• Perform patient assessments and monitor vital signs to ensure health stability.
• Provide personal care services including bathing, grooming, and meal preparation.
• Assist with patient mobility and physical therapy exercises as needed.
• Observe and document patient progress, reporting any changes to the healthcare team.
• Educate patients and families on care plans, medications, and health management strategies.
• Maintain compliance with HIPAA regulations to protect patient confidentiality.
• Collaborate with social workers and other professionals for holistic patient support.
• Offer direct support in hospice care settings, ensuring comfort for terminally ill patients.

Requirements

• Proficient understanding of HIPAA regulations to maintain patient privacy.
• Experience in hospice care or palliative settings is highly desirable.
• Strong background in social work principles to assist patients' emotional needs.
• Excellent patient monitoring skills to identify changes in health status promptly.
• Ability to observe patients closely for any signs of distress or complications.
• Capable of providing meal preparation that meets dietary restrictions and preferences.
• Demonstrated experience in caregiving roles, showcasing compassion and patience.
• Strong communication skills for effective interaction with patients, families, and healthcare teams.
