About the position

The Home & Community Support Staff position involves providing direct support to individuals with intellectual disabilities in both home and community settings. The role focuses on assisting clients in achieving personal goals, implementing community life plans, and facilitating recreational activities, all while ensuring their safety and well-being.

Responsibilities

• Provide one-on-one support to individuals with intellectual disabilities.
• Assist in the implementation of each individual's Community Life Plan.
• Accompany individuals on recreational, social, and community outings.
• Facilitate and engage individuals in leisure activities such as arts, crafts, and games.
• Provide general assistance with activities of daily living (ADLs) including personal care, meal preparation, and housekeeping.
• Complete accurate and timely documentation related to client interactions and progress.
• Maintain open lines of communication with supervisory staff and report on individual progress.
• Follow supervisor instructions in the event of a crisis to ensure safety.
• Model professionalism by demonstrating appropriate language, dress, and behavior.
• Complete mandatory annual training relevant to Human Services.
• Transport individuals in a personal or agency vehicle as needed.

Requirements

• High School Diploma or G.E.D.
• At least 18 years of age.
• Valid driver's license and insured personal vehicle.
• Ability to meet transportation requirements as outlined in the job duties.
• Experience in caregiving and working with intellectually disabled individuals (2 years required).
