Community Care, Inc. is currently seeking a reliable home health aide (HHA) to provide one-on-one care for a young adult in the Uniontown area. The HHA will be responsible for assisting with all activities of daily living and will attend school with the patient when school is in session. This position is full-time, with shifts available for 7 hours a day, Monday to Friday, ensuring no weekend work is required. The home health aide is a paraprofessional staff member trained to provide personal care and assist with daily living activities, exercise programs, and related services in the home for sick and disabled individuals when no family member can assume this responsibility. The HHA will function under the direction, instruction, and supervision of a registered nurse and/or therapist. Key responsibilities include helping the patient with personal hygiene tasks such as bathing, nail and skin care, and oral hygiene. The HHA will also prepare nutritious meals or snacks, assist with treatments like simple dressings and routine exercises, and provide basic emotional support to the patient and their household. Maintaining a clean, safe, and healthy environment for the patient is essential, as is making pertinent patient observations and recognizing emergencies. The HHA will remind the patient to take medications according to the care plan but will not administer medications themselves. Documentation is a critical part of the role, requiring the HHA to accurately read and follow the HHA Plan of Care and complete home health aide notes for each visit, submitting them to the office within the electronic medical record. Communication with the RN clinical supervisor regarding any changes in the patient's condition or home situation is vital, as is maintaining confidentiality in accordance with HIPAA regulations. The HHA must comply with infection control and safety policies, adhere to agency policies to maintain established Standards of Care, and carry out only those activities for which they have been trained and supervised. Professional development is encouraged, with a requirement to attend twelve hours of health-related continuing education per year and participate in additional staff meetings and educational sessions as appropriate.
