The Home Health Aide provides patient care services under the supervision of a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Pathologist in accordance with an established plan of care and per agency policies/procedures. This position requires population specific competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. Responsibilities

  • Calls patients and schedules visits in coordination with established plan of care, supervisor of care and the scheduling department
  • Performs patient care as directed by the care plan and patient tolerance level for that day
  • Provides a patient report to the supervising clinician assigned to the patient after each visit
  • Maintains a clean and safe environment by changing bed linens, cleaning the immediate area around the patient, patient's bathroom in relation to the patient’s personal care, light housekeeping, etc.
  • Prepares progress notes for each visit and completes necessary paperwork assignments to document patient care activities as a part of the medical record

Requirements

  • Certification by Medicare Standards prior to onset of job responsibilities and must be on the Home Health Aide Registry of the State Department of Health
  • Certification as a Home Health Aide in the State of Oklahoma, or must pass a written and skills exam to become certified
  • CPR certification
  • 1 year of nursing experience or home health aide experience
  • Previous home health experience preferred
  • Must have a current Oklahoma State Drivers License and an acceptable driving record
