About the position

The Home Health Aide (HHA) position at CenterWell involves providing direct patient care under the supervision of a registered nurse. The role focuses on assisting patients with daily living activities, medication management, and maintaining personal hygiene, while ensuring a safe and comfortable environment. The HHA is responsible for documenting patient interactions and reporting any changes in health status to ensure continuity of care.

Responsibilities

  • Provides direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
  • Assists patients with self-administered medications, ensuring correct dosages and observing the patient during administration.
  • Takes accurate temperature, pulse, and blood pressure measurements, reporting any abnormal results to the supervisor.
  • Helps patients maintain personal hygiene through bathing, grooming, and other activities.
  • Assists with feeding patients and records food and fluid intake as necessary.
  • Assists with patient toileting and manages catheter bags and ostomy care as needed.
  • Supports patient mobility, exercises, and transfers according to the Plan of Care.
  • Documents patient interactions and care provided accurately and timely.
  • Practices infection control principles to maintain a clean and safe environment.
  • Assists with light household duties such as laundry, meal preparation, and housekeeping when necessary.

Requirements

  • High school diploma or equivalent.
  • Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
  • Must meet applicable state certification requirements.
  • A valid driver's license, auto insurance, and reliable transportation are required.
  • Must be in good standing on the HHA Registry and have completed HHA/CNA course to work for a Medicare certified agency.
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant.
