About the position

The Home Health Aide (HHA) position involves providing direct patient care under the supervision of a registered nurse (RN) and according to the Aide Plan of Care (POC). The role is essential in assisting patients with daily living activities, medication management, and maintaining personal hygiene, thereby contributing to their overall health and well-being.

Responsibilities

  • Provide direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
  • Assist patients with self-administered medications, including opening bottles and checking doses.
  • Take and record vital signs such as temperature, pulse, and blood pressure, and report any abnormalities to the supervisor.
  • Help patients maintain personal hygiene through bathing, grooming, and other activities.
  • Assist with feeding patients and monitor their nutritional intake.
  • Support patients with toileting needs and manage catheter bags as necessary.
  • Facilitate patient mobility, exercises, and transfers according to the Plan of Care.
  • Document patient interactions and care provided accurately and timely.
  • Practice infection control principles to maintain a clean and safe environment.
  • Assist with light household duties such as laundry, meal preparation, and housekeeping.

Requirements

  • High school diploma or equivalent.
  • Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
  • Must meet applicable state certification requirements.
  • Valid driver's license, auto insurance, and reliable transportation are required.
  • Good standing on the HHA Registry (if applicable) and completion of HHA/CNA course for Medicare-certified agency.
  • At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant.
