Job Responsibilities: Assist patients in the maintenance of good personal hygiene grooming including bed, sponge, tub or shower baths, oral hygiene toileting, shaving patient, shampoos, dressing and other activities according to plan of care. Assist in rehabilitation which includes safe transfer techniques and ambulation, normal range or motion and positioning, assistance with use of crutches, walkers, Hoyer lifts and exercise program as instructed by the registered nurse or therapist. Plan and prepare regular and modified meals, measuring and preparing special diets in cooperation with patients and assisting with feeding patients. Report responses of feeding to the registered nurse and provide nutritional support as needed. Perform routine housekeeping tasks as related to a safe and comfortable environment for the patient. Assist with change of ostomy bags, change of colostomy bag, simple dressing, special skin care, urine testing and medication administration. Report changes in the client's conditions and needs to the registered nurse Perform other related supportive tasks Job Qualifications: Completed valid medical assessment Valid New York State HHA Certificate References upon request Proof of employment authorization and identity Ability to apply Bill of Rights and a good understanding of the basic human needs of individuals and families. Strong recreational, interpersonal and communication skills., in daily activities. Ability to work independently in a team environment.
