The PRN Home Health Aide position at CommonSpirit Health at Home involves providing compassionate care to patients in the Central Houston, South East, and South West Houston areas. Responsibilities

• Provide compassionate care to patients in their homes.
• Assist with daily living activities and personal care tasks.
• Support patients with mobility and physical activities.
• Monitor and report changes in patients' health status to the healthcare team.
• Maintain a clean and safe environment for patients.
• Document care provided and communicate effectively with team members.

Requirements

• High School Diploma or GED.
• Completion of a Certified Nursing Assistant (CNA) program (75, 120, or 150 hours).
• Must be listed in the State C.N.A. Registry.
• Successful completion of a competency evaluation program for Home Health Aide (HHA).
• One year of experience as a Certified Nursing Aide in Home Health or Hospice.
• One year of experience as a Home Health Aide, patient tech, or related experience.
