About the position

The Home Health Care Transition Coordinator is responsible for ensuring a smooth transition for patients from hospital to home health care services. This role emphasizes the importance of communication and collaboration with physicians, patients, families, and healthcare providers to facilitate appropriate care and support. The coordinator upholds the company's values of Compassion, Integrity, and Excellence while promoting positive relationships within the organization and with external stakeholders.

Responsibilities

  • Regularly meets with physicians in the hospital to discuss specific patients and provide guidance on post-acute service support.
  • Ensures continuity of care as a priority and supports the transition to home health services.
  • Educates patients and families on home health benefits and criteria for homebound status.
  • Develops collegial relationships with Ascension professionals and collaborates with clinicians to review cases.
  • Facilitates 'transition to home' planning by assessing post-discharge needs and implementing transition plans.
  • Coordinates patient care by obtaining necessary documentation and verifying demographic information.
  • Conducts follow-up on re-hospitalized home health patients and participates in mitigation strategies.
  • Ensures excellent customer service to maintain and grow business in key accounts and meets assigned quotas.

Requirements

  • Active and unencumbered Registered Nurse license in the state of employment.
  • Bachelor's degree and experience in healthcare marketing preferred.
  • Two to three years of nursing experience, preferably in a hospital or long-term care setting.
  • Experience with Home Health eligibility and admission requirements, including Medicare Home Health Benefit knowledge.
  • Strong communication skills, both written and verbal, with the ability to present information effectively.

Nice-to-haves

  • Experience with predictive analytics and data analysis skills.
  • Knowledge of risk scoring and Care Delivery Scorecard.
  • Familiarity with hospice philosophy and end-of-life practices.
