About the position

The Home Health Case Manager RN position at ViTalent involves managing the care of 20-25 patients in the Green Bay area. Responsibilities

• Manage case load of 20-25 patients.
• Conduct in-home visits for admissions, routine check-ups, recertifications, and discharges.
• Develop and complete skilled nursing care plans.
• Collaborate with interdisciplinary teams and provider offices to ensure optimal patient outcomes.

Requirements

• Current RN license in Illinois (IL).
• BLS certification.
• Minimum of 1 year of Home Health RN experience.
• Experience with EPIC charting system.
