About the position

The Clinical Coordinator at Brookestone Home Health Omaha plays a vital role in delivering personalized home health care. Responsibilities

• Review eligibility of referrals to Home Health
• Provide education to staff on OASIS, PDGM, and Home Health regulations
• Ensure compliance with health regulations
• Interact positively with patients and family members
• Support the Administrator and intake team in daily operations

Requirements

• Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist qualification
• Knowledgeable in OASIS, PDGM, and Home Health regulations
• Current CPR certification
• High personal integrity and a caring attitude
