About the position

The Clinical Coordinator at Brookestone Home Health Omaha plays a vital role in delivering personalized home health care. This position involves working closely with patients, families, and team members to ensure high-quality care and compliance with health regulations. The Clinical Coordinator will review referrals, provide education to staff, and support the overall mission of helping patients recover in their own homes.

Responsibilities

  • Review eligibility of referrals to Home Health
  • Provide education to staff on OASIS, PDGM, and Home Health regulations
  • Ensure compliance with health regulations
  • Interact positively with patients and family members
  • Support the Administrator and intake team in daily operations

Requirements

  • Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist qualification
  • Knowledgeable in OASIS, PDGM, and Home Health regulations
  • Current CPR certification
  • High personal integrity and a caring attitude
