About the position

The Home Health Clinical Triage Team Lead is responsible for overseeing after-hours, holiday, and weekend call coverage for the clinical support team. This role ensures appropriate staffing levels to meet business needs and maintain customer satisfaction while providing high-quality, patient-focused care. The Team Lead will also handle patient calls, assess concerns, and coordinate nursing care as needed, while ensuring compliance with triage protocols and documentation standards.

Responsibilities

  • Provide after-hours, holiday, and weekend call coverage for the clinical support team.
  • Oversee and manage the department shift schedule and workload for the assigned team.
  • Ensure appropriate staffing levels to meet business needs and customer satisfaction.
  • Support Clinical Support operations and ensure adherence to triage protocols and best practices.
  • Step into the role of a Clinical Support Specialist in emergent situations as needed.
  • Handle calls from patients and family members, assessing concerns and complaints accurately.
  • Document calls received in the electronic medical records system in a timely manner.
  • Coordinate with on-call nurses to deliver high-quality nursing care and schedule assessments as required.
  • Maintain education on updates and processes in the electronic medical record and relevant regulations.
  • Facilitate orientation of new personnel as assigned.

Requirements

  • Must be a licensed Registered Nurse in good standing with no restrictions in all states of practice.
  • Compact licensure preferred.
  • Two (2) years' experience as a Registered Nurse with at least one year of direct patient care in a hospice, home health, or oncology setting.
  • Previous leadership experience in Home Health of at least one (1) year preferred.
