About the position

The Home Health & Hospice Social Worker LMSW / LCSW is responsible for delivering quality Medical Social Work services to patients and their families, ensuring care aligns with the agency's philosophy and standards. This role involves comprehensive psychosocial assessments, effective communication with interdisciplinary teams, and the development and evaluation of care plans for patients across various age groups.

Responsibilities

• Provides care and counseling to sick palliative care and hospice patients and families.
• Communicates effectively with members of the interdisciplinary team (IDT) to promote a team-centered approach to care.
• Attends Hospice IDT meetings and other relevant care meetings.
• Performs comprehensive psychosocial assessments addressing the relationship between medical conditions and psychosocial functioning.
• Meets social work goals related to patients' medically related psychosocial problems through effective problem resolution.
• Provides education and counseling to patients and their families regarding care needs.
• Develops, implements, and evaluates the plan of care, making necessary revisions.
• Addresses all problems in the plan of care and implements orders in a timely manner.
• Makes recommendations for additional home health care and community services as needed.
• Documents patient visits accurately and timely, using appropriate documentation tools.
• Communicates effectively and tactfully with clients, considering their age, cultural diversity, and needs.
• Participates in the agency's Quality Improvement (QI) program as required.

Requirements

• Master's degree in Social Work and/or Board Eligible.
• Two years of experience in social work in a health care setting.
• Current CT State LMSW Licensure (Licensed Masters of Social Work).
• Maintains a current Driver's License and auto insurance.
