Home Health & Hospice: Team Coordinator

About the position

The Team Coordinator at Lifespark Group LLC is responsible for managing the daily operations of the Home Health team, ensuring effective scheduling, quality compliance, and financial performance. This role involves coordinating with clinicians, handling customer service inquiries, and supporting the Clinic Director in administrative tasks. The position is crucial for maintaining the Lifespark Experience and enhancing team efficiency.

Responsibilities

• Coordinate the day-to-day workflow of the Home Health team.
• Manage scheduling for clients and employees.
• Provide customer service by answering phones and resolving issues.
• Coordinate with clinicians and billing manager for accurate authorizations and paperwork.
• Ensure compliance with organization policies regarding visits and documentation.
• Foster a team environment by seeking input for process improvements.
• Identify opportunities for technology to enhance business processes.
• Serve as the front line for incoming phone calls for the team.
• Perform data entry for referrals and track spreadsheets as needed.
• Act as the administrative point of contact for the Clinic Director.
• Analyze data to understand workload demands and timelines.
• Continuously monitor schedules to meet productivity goals.
• Provide coverage to other departments as needed.
• Perform other duties as assigned.

Requirements

• At least three years of experience in Home Health/Community scheduling.
• Home Health and/or Hospice experience preferred.
• Must be at least 18 years of age and a high school graduate or equivalent.
• HCHB (Homecare Homebase) experience preferred.
• Billing/Customer Services experience preferred.
• Associate's degree preferred.
• Experience as a Health Unit Coordinator (HUC) is a plus.
