About the position
The Home Health Intake Clinician (LPN) at Franciscan VNS Home Health is responsible for facilitating patient care by managing intake processes, coordinating with various healthcare providers, and ensuring compliance with insurance and billing requirements. This role involves direct communication with patients, families, and referral sources to ensure continuity of care and appropriate service delivery.
Responsibilities
- Assist billing and tracking nonpaid claims, including researching information and documentation for insurance-related billing problems.
- Investigate and determine initial patient benefits coverage for requested services, including Medicare, Medicaid, private insurance, and other funding sources.
- Assist patients, families, physicians, and clinicians via telephone with clinical issues, problems, and questions.
- Work collaboratively with referral sources to facilitate continuity of care and appropriate care.
- Act as a liaison with pharmacy, medical supplies, DME, and other vendors providing care to VNS patients.
- Accept and process physician orders via telephone.
- Communicate with referral sources to gather necessary information for patient care continuity.
- Complete appropriate referral forms and enter information into the computer.
- Develop and implement clinical review systems for all pay sources, including admissions and continued stay reviews.
- Document interventions taken and orders received as indicated.
- Obtain managed care and insurance authorizations to determine applicable coordination of benefits.
- Schedule patient visits for field staff to ensure compliance with the plan of care.
Requirements
- Associate's Degree or Diploma from an Accredited Licensed Practical Nurse Program Required
- Licensed Practical Nurse (LPN) License Required
- 2 Years Home Health Care Experience Preferred
- Driver's License Required
- Auto Insurance Required
