About the position

We are seeking a full-time Licensed Practical Nurse (LPN) to join our dedicated team in West Orange, NJ. This role focuses on providing high-quality clinical services within the scope of nursing practice, ensuring effective communication with patients, families, and healthcare team members. The LPN will play a vital role in patient care, documentation, and quality improvement, all while fostering a culture of caring and support within the community.

Responsibilities

• Provides high quality clinical services within the scope of practice as defined by state laws.
• Observes and reports the patient's nursing needs to the supervising RN.
• Communicates effectively with physicians, patients, family members, and agency staff regarding patient care issues.
• Documents patient visits thoroughly and maintains an accurate medication listing.
• Participates in the performance improvement plan to reduce hospitalizations and improve patient outcomes.
• Attends and participates in all meetings, including case conferences, and is punctual.
• Maintains a positive attitude and is willing to help patients and the agency in any way possible.
• Adheres to the agency's Episode Management process and performs all other duties as assigned.

Requirements

• Current LPN licensure in the state of practice.
• Current Driver's License and vehicle insurance.
• Access to a dependable vehicle or public transportation.
• CPR Certification required.
