About the position

The Home Health LVN in Office Case Manager role at Optum involves managing clinical operations within the home health department. The position requires coordination of various clinical disciplines, ensuring quality patient care, compliance with regulations, and effective staff management. The LVN will work closely with the Assistant Clinical Team Manager to oversee staff performance, patient care plans, and operational improvements while maintaining a professional and ethical approach to healthcare delivery.

Responsibilities

  • Provides direction and leadership to designated Agency personnel to ensure quality and continuity of services.
  • Ensures compliance of health care staff with federal and state regulations and agency policies.
  • Executes clients' plans of care as written and reassesses client needs as necessary.
  • Participates in orientation and training of clinical and paraprofessional staff.
  • Monitors staff performance and ensures timely supervisory requirements are met.
  • Demonstrates effective leadership and problem-solving skills.
  • Coordinates patient services and interdisciplinary communication.
  • Receives and coordinates patient referrals from various sources.
  • Participates in interdisciplinary care conferences and management functions.
  • Monitors documentation to ensure accuracy and compliance with payer guidelines.
  • Investigates patient/family complaints and takes corrective action as needed.
  • Makes home visits to provide patient care as necessary.
  • Recognizes and responds to life-threatening situations appropriately.
  • Participates in accreditation and licensing endeavors as required.
  • Assists in developing Performance Improvement Plan reports and participates in quality improvement activities.
  • Educates clinical staff on documentation standards and patient care.
  • Monitors assigned cases for compliance with third-party payer requirements.
  • Attends required in-service programs to maintain home care knowledge.
  • Demonstrates effective time management and communication skills.
  • Promotes agency philosophy and maintains patient confidentiality.
  • Participates in on-call duties as assigned.

Requirements

  • High school diploma or equivalent.
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Current unrestricted license in Texas as a Licensed Vocational Nurse.
  • Valid Texas Driver's License and personal auto insurance coverage.
  • Current CPR certification.
  • 2+ years of clinical experience in community home health or medical/surgical settings.
  • Experience with electronic medical record applications.
  • Thorough knowledge of nursing practice and regulations regarding home care.
  • Basic computer skills including Microsoft Word and Outlook.
  • Excellent observation, communication, problem-solving, and nursing skills.
  • Ability to perform independent nursing practice and deliver care effectively.
  • Ability to lift and transfer patients, and perform physical tasks as required.

  • Experience in a leadership or supervisory role within a healthcare setting.
  • Familiarity with home health care regulations and standards.
  • Advanced computer skills or experience with specific healthcare software.
