About the position
The Home Health LVN/LPN position is a contract role focused on providing home-based healthcare services. The role requires a licensed vocational nurse or licensed practical nurse to work day shifts, primarily in Needham, Massachusetts, with a commitment to patient care in a home health setting. Responsibilities
- Provide home-based healthcare services to patients.
- Ensure compliance with healthcare regulations and standards.
- Administer medications and treatments as prescribed.
- Monitor patient health and report any changes to the healthcare team.
- Maintain accurate patient records and documentation.
- Assist with patient mobility and daily living activities.

Requirements
- Valid MA LVN License is required.
- Minimum of 2 years of experience in Home Health is required.
- Certifications required: BLS (Basic Life Support).
- Experience with OASIS is required; familiarity with Homecare Homebase (HCHB) is preferred.
