About the position
Aveanna Healthcare is seeking a Social Worker to join its home health care team, responsible for evaluating, coordinating, and administering social work services to patients and their families. This role involves understanding the social and emotional factors affecting health, developing care plans, and ensuring effective service delivery through collaboration with other caregivers and community resources.
Responsibilities
- Assist clients in understanding significant social and emotional factors related to their health problems.
- Develop a plan of care based on clinical diagnosis, client and physician input, including medical and social interventions and measurable goals.
- Arrange for the implementation of care plans by linking clients to community medical and health resources.
- Review client progress to assess the effectiveness of services in achieving rehabilitation goals.
- Use appropriate medical, social, diagnostic, and psychological terminology in documentation.
- Investigate and address client or employee concerns and complaints appropriately.
- Instruct clients, family members, and primary caregivers to ensure optimal functioning levels for each client.
- Coordinate agency services by orienting and training other caregivers to meet client needs according to agency policies.
- Ensure effective coordination of home care services through timely documentation and transfer of medical data to relevant parties.
- Provide input to the Medicare Director regarding agency policies and practices related to client services.
- Maintain accurate and comprehensive client data.
Requirements
- Current state license as required by state law and regulation.
- Masters Degree in Social Work.
- Minimum of at least one year of medical social work experience.
- Valid CPR certification.
