About the position

Advocate Aurora Health is seeking a Home Health Occupational Therapist to provide therapy services in patients' homes. This role involves working independently to assess and address the mobility, safety, communication, and equipment needs of patients, while also establishing strong relationships with patients and their families. The position is part-time, requiring 32 hours per week, and offers a supportive environment within a reputable healthcare organization.

Responsibilities

• Demonstrates proficient use of all home-based therapy equipment and assistive/adaptive devices.
• Assesses, plans, implements, and evaluates occupational therapy treatment programs for home care patients.
• Facilitates the progression of patients toward optimal levels of self-care in their home environment.
• Communicates professionally both verbally and through written reports, ensuring strong documentation skills.
• Develops and maintains restorative relationships with patients and families throughout the healthcare continuum.

Requirements

• Licensed in the State of Illinois (or eligible for licensure).
• Completed 1 year of clinical experience or strong clinical affiliation experience.
• Graduated from an Occupational Therapy curriculum accredited by the American Occupational Therapy Association.
• Ability to work independently in a non-acute setting, such as patient homes.
• Demonstrates good time management and self-organization skills.
