About the position
LHC Group is seeking a full-time Home Health Physical Therapist to join their team in Verona, New Jersey. This role focuses on providing high-quality, patient-centered care in a home health setting, emphasizing the importance of functional mobility, pain management, and overall well-being. The Physical Therapist will work collaboratively with healthcare teams to develop and implement effective treatment plans, ensuring that patients receive the best possible care in their homes.
Responsibilities
- Evaluate the patient's functional status and physical therapy needs.
- Assist the physician in the development of the therapy plan of care.
- Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating members of the healthcare team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
- Current Physical Therapy licensure in the state of New Jersey.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
