About the position

The Home Health Physical Therapist will provide skilled and compassionate care to patients in home hospice and palliative care settings in the Houston, TX area. This role focuses on helping and guiding patients and their families through end-of-life care, ensuring comfort and quality of life during this critical time.

Responsibilities

• Provide physical therapy services to patients in home hospice and palliative care settings.
• Assess patients' physical conditions and develop individualized treatment plans.
• Work with patients of all ages, including pediatrics, to improve mobility and manage pain.
• Educate patients and families on physical therapy techniques and exercises.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.

Requirements

• Current Physical Therapist license in the state of Texas.
• Experience in home health care settings.
• Ability to work with patients of all ages, including pediatrics.
• Strong communication and interpersonal skills.

Nice-to-haves

• Experience in hospice or palliative care settings.
• Bilingual skills to communicate with diverse patient populations.
