About the position

LHC Group is seeking a dedicated Home Health Physical Therapy Assistant to provide care for clients in the Needham area. This role emphasizes building trusted relationships with patients and their families while delivering high-quality physical therapy services in a home health setting. Responsibilities

• Perform treatment and provide exercise instruction and patient education aimed at improving the patient's well-being.
• Follow all state-specific laws governing the provision of physical therapy in home care, adhering to the treatment plan defined by the supervising PT.
• Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional activities, ADL, transfer, safety, and prosthetic training.
• Observe, record, and report the patient's response to treatment and changes in condition to the supervising PT, nurse supervisor, and/or physician.
• Coordinate care with other members of the healthcare team as appropriate.
• Instruct patients, families, caregivers, and other healthcare team members in therapy areas within the scope of the physical therapy assistant.

Requirements

• Current licensed, registered, or certified to assist in the practice of physical therapy in Massachusetts.
• Current CPR certification is required.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
